ATHLETIC CHECKLIST

We are thrilled to hear your student is interested in playing a sport this school year! Here is everything you need to know to start the process:
Athletic Checklist:
Before a student can participate in any MCS sport, they must first complete the following:
1) Sports Physical Form
2) Academic release of eligibility
3) Athletic fee paid
4) School tuition account paid up to date.
An Athletic Release Packet must be completed for each sport played for the 2025-2026 school year.
Middle School Athletic Fee:
The non-refundable, non-transferable athletic fee is:
1st Sport $100.00 - 2nd Sport $75.00 - 3rd Sport $50.00
Please make payment through the FACTS Family Portal.

All forms must be turned into the Athletics department and payment must be received before the sport season starts.

High School Athletic Fee:
The non-refundable, non-transferable athletic fee is:
$250.00 / Football - $200.00 / for all other sports
